What we collect
When you complete our questionnaire we collect what you enter: the borough and ZIP code, who the care is for, whether the person is enrolled in a managed long-term care plan and which one, whether they receive home care, the kinds of daily help they need, memory concerns, language, transportation needs, preferred days, and your name, phone number and email. We also collect technical data such as your browser type, a hashed form of your IP address, and advertising click identifiers.
How we use it
To tell you whether a social adult day care program may be a fit, and, with your consent, to share your request with up to three registered social adult day care centers in your area so they can contact you. Centers pay us a flat marketing fee; the fee does not depend on whether you enroll anywhere. We do not sell your information to data brokers, and we do not share it with Medicaid managed long-term care plans.
Contact and consent
If you consent on the form, we and the centers named in the consent may contact you by phone, text and email, including with automated technology. Reply STOP to any text to opt out, or email us at the address below. Consent is not a condition of any service.
Who we are not
We are not a Medicaid managed long-term care plan, not a day care center, and not affiliated with NY Medicaid Choice or the New York State Department of Health. Questions about managed long-term care enrollment in New York go to NY Medicaid Choice at 1-888-401-6582.
Retention and security
We keep questionnaire records for as long as needed to run the service and to meet record-keeping obligations, then delete or anonymize them. Data is stored with access controls and encryption in transit.
Your choices
You may ask us to correct or delete your information, or to stop contacting you, at any time by writing to Adult Care Network. We will honor requests within 30 days where the law allows.
